Seniors Day Club
Seniors Day club is a service provided by St Carthage’s Community Care with funding received from the Department of Ageing, Disability and Home Care through the Home and Community Care (HACC) program.
Seniors Day Club provides centre-based activities to frail older people and people with disabilities residing in the Lismore LGA out of the Goonellabah Community Centre.
Seniors Day Club is available:
Mondays from 10am – 2pm. Transport is available for people residing in the Lismore LGA.
Wednesdays from 10am – 2pm. Transport is available for people residing in the Lismore LGA.
Fridays from 10am – 2pm. Friday Seniors is Italian-specific for people from an Italian background. Transport is available for people residing in the Lismore LGA.
Who is eligible?
To be eligible for Seniors Day Club, clients will:
- be aged 65 years and over (or 50 and over if from an indigenous background);
- reside in the Lismore LGA.
How do I refer to this service?
Referral can be made by telephone, fax or in person – see below for contact details
Carers, clients and family members can contact the Respite team on 6620 0000.
Referral is made via the CIARR for service providers.
Assessment and Prioritising
When a telephone referral is received, a member of the Respite Team who answers the call will complete the Intake/Assessment Form. This section will determine eligibility of the client and the services they require. The staff member will inform the referrer immediately of eligibility and will follow this up with written confirmation. The Program Officer or Coordinator will then contact the Carer to complete the assessment process. Once the Intake / Assessment process is completed, the respite care plan will be developed for the client.
