Extended Aged Care at Home (EACH)

Extended Aged Care at Home Packages are provided by St Carthage’s Community Care with funding received from the Department of Health & Ageing. An Extended Aged Care Package is a planned and coordinated program of community care services individually designed to assist people with complex care needs, who are eligible for nursing home care but choose to remain in their own homes. 

 EACH packages are available to assist any eligible person who requires assistance relating to

·        Case Management

·        Monitoring Medications/Treatments

·        Complex nursing

·        Social Support

·        Personal Care

·        Dressing / Undressing

·        Continence Management

·        Communication

·        Mobility / Transfer / Transport

·        Nutrition

·        Behaviour/Cognition

·        Domestic Assistance

·        Minor Home Maintenance


Who is eligible?

To be eligible for an  Extended Aged Care Package a client must be assessed by a member of the Aged Care Assessment Team.

 How Do I Refer to this service?

Referral may be made by you, a relative, friend, hospital, Doctor, ACAT or Community Service Providers.

How much will it cost?

An affordable fee for service applies.  This fee is 17.5% of the single maximum pension excluding rental assistance and pharmaceutical allowance.  Your EACH Coordinator will discuss this with you at the initial interview.
