Community Aged Care Packages (CACP)

 

Community Aged Care Packages are provided by St Carthage’s Community Care with funding received from the Department of Health & Ageing. A Community Aged Care Package is a planned and coordinated program of community care services individually designed to assist people with complex care needs, who are eligible for low level care but choose to remain in their own homes.  Community Aged Care Packages are available to assist any eligible person who requires assistance relating to

·        Case Management

·        Showering / Bathing

·        Dressing / Undressing

·        Mobility / Transfer / Transport

·        Meals (provision & preparing)

·        Laundry / Light Housework

·        Monitoring Medications

·        Short term In-Home Respite

·        Socialisation

·        Minor Home Maintenance


 
Who is eligible?

A client must be assessed as being eligible for a Community Aged Care Package by a member of the Aged Care Assessment Team.

 

How Do I Refer to this service?

Referral may be made by you or a relative, friend, hospital, Doctor, ACAT or Community Service Providers on your behalf.

 
How much will it cost?

An affordable fee for service applies.  This fee is 17.5% of the single maximum pension excluding rental assistance and pharmaceutical allowance.  The CACP Coordinator will discuss this with you at the initial interview.
